Uniform Information - PreK students wear the P.E. uniform

Suggested Summer Work:
- Write the first name with only the first letter uppercase. Example: Colleen, not COLLEEN
- Self-care: putting on and taking off sweatshirts, coats, shoes (velcro), zipping
- Toileting skills (wiping, flashing, washing hands)
- Opening snack and lunchbox items
Required Health Forms - due by August 1
- MD State Record of Physical Examination
- Immunization Record (provided by a physician)
- Dental Form
- All About Me Form
- Health Practitioner Emergency Form
- Maryland Lead Testing Form
Optional Health Forms (all mediation order forms must be signed by the provider and parent)
