Suggested Summer Work:

Write the first name with only the first letter uppercase. Example: Colleen, not COLLEEN
Self-care: putting on and taking off sweatshirts, coats, shoes (velcro), zipping
Toileting skills (wiping, flashing, washing hands)
Opening snack and lunchbox items

MD State Record of Physical Examination
Immunization Record (provided by a physician)
Dental Form
All About Me Form
Health Practitioner Emergency Form
Maryland Lead Testing Form

Optional Health Forms (all mediation order forms must be signed by the provider and parent)

1st Day School Supplies (coming soon)

SJS-Fullerton has partnered with 1st Day School Supplies to simplify back-to-school supply shopping! Organized by grade, these kits include most* of the school supplies St. Joseph School-Fullerton requires.

Ordering through 1st Day is completely optional.
Orders must be placed before March 1 and July 1.
All personal items will be shipped to your home for labeling, while all school items, such as copy paper, tissues, paper towels, etc., will be shipped directly to the school.

*Please review the school supply list carefully. Some required items are unavailable through 1st Day and must be purchased separately.
